A right-sided subcutaneous implantable cardioverter defibrillator in a patient with congenital heart disease.

چکیده
A young woman with congenital heart disease including dextrocardia presented to us following an out-of-hospital ventricular fibrillation (VF) arrest. She had a dual-chamber pacemaker inserted already for bradycardia. Therewere concerns over the risk of baffle occlusion due to multiple transvenous wires. She received an entirely right-sided subcutaneous implantable cardioverter-defibrillator with successful defibrillation tests.

Implantable cardioverter-defibrillator in a patient with dextrocardia situs inversus
Background: Dextrocardia is a congenital anomaly, which may have coexistent coronary artery disease (CAD), arrhythmias and conventional indications for device therapy. However, the implantation of transvenous leads can be technically challenging and the approach needs to be tailored to the patient's individual anatomy. Case presentation: A 54-year-old male with dextrocardia situs inversus and i...
متن کاملThe Subcutaneous Implantable Cardioverter- Defibrillator as Part of Dual Device Therapy in Complex Congenital Heart Disease
We report the case of a 44-year-old with unrepaired complex congenital heart disease (CHD), a pre-existing permanent pacemaker, and a subcutaneous implantable cardioverterdefibrillator (SICD) implanted for secondary prevention. Right parasternal electrode positioning and step-down defibrillation testing were integrated. Application of the SICD for device therapy in CHD patients remains quite li...
متن کاملRight Parasternal Lead Placement Increases Eligibility for Subcutaneous Implantable Cardioverter Defibrillator Therapy in Adults With Congenital Heart Disease.
BACKGROUND The subcutaneous implantable cardioverter defibrillator (S-ICD) provides an attractive option for patients with congenital heart disease (CHD) in whom a transvenous defibrillator is contraindicated. Given the unusual cardiac anatomy and repolarization strain, the surface electrocardiogram (ECG) is frequently abnormal, potentially increasing the screen failure rate. METHODSANDRESULT...
